Scholarship Form: Financial assistance is available for all memberships and programs. Filling out this application does not guarantee a scholarship. Scholarship Application in Spanish
Waiver of Liability: All program attendees and members must have a liability waiver in order to participate

All Day Summer Care Form: Register your child now and they will enjoy summer fun all week long at Proehlific Park!
Membership Cancellation: In order to cancel your membership you must stop by Proehlific Park in person.
