Waiver of Liability : All program attendees and members must have a liability waiver in order to participate

After School Care Application : Proehlific Park's Afterschool program is a great place for children 7 years old and up. To register submit this form along with deposit and registration fee to Janet Clark.
All Day Summer Care Form : Register your child now and they will enjoy summer fun all week long at Proehlific Park!
Special Programs Applications: This application is good for all special programs offered at Proehlific Park that do not have a specific application.
Membership Cancellation: In order to cancel your membership please complete and submit the following form. This form will count as your 30 days notice to cancel your membership.
